Welcome to Fontana High School! We at FOHI feel our school is the best place in the world for secondary education. The FOHI curriculum covers a broad spectrum of academic, vocational, and business areas. In order to make your enrollment a pleasant and easy experience, please follow the guidelines below and use them as a checklist.

1. Pick up an enrollment packet from one of the three counseling centers:
- 9th and 10th Grade: E Building Guidance (909)357-5500 ext. 6865
- 11th Grade: PO 3 Guidance (909) 357-5500 ext.6164
- 12th Grade: Administration Guidance (909)357-5500 ext. 6553

A PARENT OR GUARDIAN MUST ACCOMPANY STUDENT.


2. If your child speaks another language other than English or is bilingual, he/she will need to take an assessment after meeting with the counselor. The Assessment Center is located in the Central Enrollment Center Building A #A24.


3. If your child has Special Education Placement, please call Irene Gomez at (909)357-5000 ext. 7332 or go to Window 3 at the Central Enrollment Center Building A. Once you have met with Ms. Gomez, please call the offices as listed above for an enrollment appointment.


4. See the FOHI nurse, located in Building E Room 109, to have your child's shot records verified. Immunizations are required by law. A parent/guardian must accompany student.


5. Call the appropriate counseling office for an appointment to enroll your student(s). Due to limited seating, we ask that only the parent/guardian and student attend the enrollment appointment.


6. Once you have been given an appointment with a counselor, please be sure to bring the following items:
- Check out grades from the last school of attendance (after 20 days). THIS IS MANDATORY FOR SENIORS.
- Transcript (may be unofficial) from the last school of attendance. THIS IS MANDATORY FOR SENIORS.
- Proof of Residency in parent/guardian's name (Rental Agreement; Escrow Instructions; Utility Bill; home telephone bill within the last 30 days only). If the bill's are not in the parent/guardian's name, the person with whom the family lives with must be present at the time of the appointment along with the parent/guardian, to fill out the Declaration of Parental Residency Form. The child(ren) must be living with the legal parent or legal court-appointed guardian in order to enroll in the Fontana Unified School District. Court documents must be present at the time of the enrollment appointment. If there are no court papers, the guardian & student must make an appointment with an Administrator, prior to the enrollment appointment, to sign a Caregiver Form. (Education Code 48200).
